Forest took the lead as defender Willy Boly netted against his former side as he slotted home from close range in the 18th minute.
However, Wolves responded in the second half and in the 64th minute, Mexican striker Raul Jiminez tapped in from on-loan Atletico Madrid attacker Matheus Cunha‘s low cross.
However, neither side were able to find a winner, and the tie had to be settled on penalties.
After both sides missed their first two spot kicks, the next six found the back of the net before Henderson made the crucial have to give forest the key to the next round.
