Play was stopped for around a minute as the flare was cleared after landing next to the Manchester United goalkeeper.
Fortunately, Onana was not struck by the pyrotechnic, which fire marshals removed from the pitch.
After a brief pause in the action, referee Andy Madley allowed the match to restart.
Onana had been spectacularly beaten just moments earlier.
Phil Foden cut in from the right before bending a left-footed strike into the top corner to level the match.
The goal was his 10th in the Premier League this season, putting him just one behind his tally from last term.
It also cancelled out his England team-mate Marcus Rashford’s opener.
The United man had opened the scoring after just eight minutes with an impressive long-range strike of his own.

City are looking to move back within a point of leaders Liverpool, who snatched a late victory at Nottingham Forest on Saturday.
However, they were not helped by a string of missed chances from Erling Haaland.
The forward’s clinical finishing deserted him as he produced one of the worst misses in Premier League history during the first half.
Haaland was left with the goal gaping but lifted his effort over the bar from just a yard out as City chased an equaliser.
