It will focus on paramedics Jan, Iain, Sah and Teddy, who are pushed to breaking point whilst working under pressure.
Bosses have planned the episode to shed light on the effect ambulance queues has on the hospital, its patients, and the front-line medical staff themselves as it reflects real life.
The episode is set over four days and shows the pressures and tasks the fictional NHS workers have to deal with.
Casualty’s executive producer Jon Sen said it seemed “only natural” to use improvisation to “capture the truth of the world”.
Steve Hughes, Director of the episode said: “The actors improvised all their dialogue and we shot everything like a documentary to give it a grounded, naturalistic feel.
“It was vital to make this episode feel as authentic as possible to highlight the challenges paramedics, and the NHS as a whole, face on a daily basis.”
He added that it “was scary for the actors at first, like they were walking a tightrope without a net”.
Meanwhile, upcoming scenes of the drama will see Jan tested.
She has enjoyed a short break from her role as she continues to struggle but upon her return, her good mood is soon tested when one of the regulars, Robbie, is found drunk and is refusing to co-operate and return to the ED.
Wading through her frustrations, she convinces Robbie to get into the ambulance along with Teddy and the next emergency call soon comes rushing in.
The improvised episode will air this Saturday, 10 December, at 20:10 on BBC One and BBC iPlayer.
